Our In-Home Supports program provides support to individuals living in their own homes. Whether you live alone, with your family, with a partner, or with friends, we assess your strengths and needs, and where applicable, those of your family, to determine the best way to support your independence and quality of life within your home and community.
Who We Support
This program is designed for:
- Children, adolescents, and adults with developmental disabilities and related conditions who remain with their families or natural caregivers.
- People of all ages, with services considered on a case-by-case basis for those under 18.
Our Approach
We offer flexible, creative, and dynamic support services that can be short-term or ongoing, depending on the needs and choices of the individual and/or family. Each Individualized Service Plan (ISP) is tailored to the unique needs, preferences, and goals of the person receiving support. Our services range from drop-in support for those living independently to comprehensive plans that include training and activities to promote skills, community involvement, and maximum independence.
Individualized Service Plan (ISP)
Each person receiving in-home support services has an ISP that specifies the type and pattern of supports needed. The ISP is:
- Developed to encourage the highest possible level of independence in self-help and daily living activities.
- Directed by the individual served or their family, ensuring it meets their needs, preferences, and goals.
- Outcome-oriented and designed to foster physical and social integration into the community.
Our team approach ensures that the ISP is established, implemented, and monitored regularly. We update and revise the plan as needed, in collaboration with the individual and their family, to ensure it continues to meet their evolving needs and preferences.
